Registration is from 9:00 AM - 10:30 AM
Children: in Green Auditorium.
Youth: in Hutto Auditorium.
Make sure you bring:
- Music and Arts Week 2012 Image Release and Consent Form (In the Camper Registration Form packet)
- The 5 forms from the new Sumatanga Health Packet (available at
http://www.musicandartsweek.org/forms):
- Camper Health Form (completed & signed by the camper's physician)
- Individualized Standing Orders for Over the Counter and Prescription Medications (completed and signed by parent and physician)
- Camp Sumatanga Arrival Day Health Status Update (to be completed ON JULY 2, 2012)
- Camp Sumatanga Head Lice Policy (to be completed ON JULY 2, 2012)
- Camper Checkout Form
